Long waiting times for medication services remain a major challenge in hospital
pharmacy services in Indonesia. At RSUD Mas Amsyar Kasongan, the waiting time for noncompounded medications reached 38 minutes and compounded medications 73 minutes,
exceeding the National Quality Indicator standards (30 minutes for non-compounded
medications and 60 minutes for compounded medications). This study aims to analyze
and implement Lean Hospital Management to optimize medication waiting times at the
hospital pharmacy. This mixed-methods action research was conducted over four months,
involving pre-intervention, intervention, and post-intervention phases. The sample
consisted of 14 pharmacy staff, 30 patients, and 120 prescriptions (60 compounded and
60 non-compounded). Data analysis employed descriptive statistics, comparative tests,
and thematic analysis. The results showed a significant reduction in medication waiting
times to 3–5 minutes, an increase in National Quality Indicator compliance from 60% to
96%, and an improvement in patient satisfaction to 83.4%. Waste identification revealed
waiting, overprocessing, overproduction, defects, and unnecessary transportation as the
primary sources of inefficiency. Lean Hospital Management proved effective in improving
operational efficiency, service quality, and patient satisfaction. The study recommends
strengthening pharmacy information systems, enhancing staff competency, and
implementing continuous monitoring
